The details:

During his rookie F1 season, Jos Verstappen, then driving for Benetton, pulled into the pits for a routine stop. This was the first season since 1983 that in-race refueling was permitted.

• The Cause: A filter in the fuel rig, intended to slow the fuel flow, was reportedly removed by the team in an attempt to speed up the pit stop. As fuel was being dispensed, a faulty connection or a surge caused fuel to spray onto the hot car.
